Player Consent & Liability Waiver
Effective Date: 26 April 2026
This agreement is entered into with Atletas India Sports and Entertainment OPC Private Limited (AISE).
1. Voluntary Participation
I confirm that participation is voluntary and at my own risk.
2. Medical Fitness
I confirm that I am medically fit to participate.
3. Assumption of Risk
I acknowledge risks including injury, illness, or unforeseen incidents.
4. Liability Waiver
I release AISE from any liability arising from participation.
5. Medical Consent
AISE may arrange medical assistance if required.
6. Media Consent
I grant AISE rights to use my images, videos, and performance data.
7. Code of Conduct
I agree to follow all rules and maintain discipline.
8. Parent/Guardian Consent
Required if participant is under 18.
